·   All personnel shall be undergone for Contractor/Company Safety induction and required necessary training such as H2S awareness prior to enter process areas
·   All work shall be carried out with approved appropriate work permit, Mandatory for the permit receiver and issuer to conduct site survey and inspection prior to release work permit. 
·   Specific work permit shall be obtained prior to start the activity. Content of work permit such as actual location, activity and safety measure shall be discussed to all involve person prior to start the activity during TBM.
·   All access ways shall be properly identify and maintained in good condition
·   Contractor shall allocate dedicated person to conduct regular housekeeping in each designated location.
·   All tools and materials shall be kept in approved designated storage area.
·   All cable shall be properly arrange and lay away from possible access way.
·   Cable shall be installed overhead as possible and applicable on the working area.

·   Task supervisor shall ensure all workforces are received actual and specific safe instruction prior to start the work.
·   Heavy and bulky tools shall not be manually lifted.
·   Lifting area shall be barricade and provided with appropriate barricade and signage and should be free person in line of fire.
·   Appropriate and inspected lifting tools and materials shall be provided in all manual lifting activity such as, pulley, rope and scaffold or lifting basket.
·   Task supervisor to ensure all damage and worn out lifting tools and materials shall be discard from site.
·   Task supervisor shall identify or provide appropriate anchorage point for the pulley.
·   All person involve in the activity shall be provided with appropriate safety hand gloves to protect from sharp object of the materials as well as maintaining proper grip of the rope while lifting.   
·    Task supervisor is mandated to closely monitor and correct all unsafe condition and action of the team during path of the task.

·   Appropriate PPE shall be provided to all involve workforce such as, hard hat, FRC coverall, appropriate safety gloves, safety shoe and face and eye protection (safety glass and face shield) 
·   A full body harness shall be worn by the team while on the scaffold with more than 1.8 meter elevation.
·   All scaffolding shall be erected, inspected and tag by competent persons as per client procedure and standards.
·   All persons shall be undergone proper working at heights training and obtained proofs such as training card or working at heights stickers mounted on their helmets prior to work.
·   All materials and tools shall be kept on safe portion at working platform; all tools shall be provided with restraining rope.
·   Close monitoring and supervision of workers
·   Work area must have sufficient clearance and remove any obstructions
·   Use only the right tools for the job, never use make shift tools or defective tools
·   Only experienced and competent person to do the job, focus on the work and avoid any distractions.
·   Pre-check up of equipment prior to use, ensure equipment is in good condition prior to start activity
·   Secure all tools and equipment when working at heights to restrain from falling, bolts and nuts shall be secured in a covered container to avoid falling below.
·   Only authorized and competent person to use bolt tensioning machine
·   Equipment must be properly maintained at all times

·   All scaffolding structure shall be erected, inspected and tag by competent person in accordance with client procedure and rules.
·   Scaffolding platforms shall used only as his purpose, pipe and other materials shall not allowed to stock or rest on the platform.
·   Task supervisor shall ensure untagged or inspected scaffold shall not be used.
·   Electrical extensions, power tool and welding cables shall be inspected and obtained with current monthly color code and neatly arranged and hung on insulated-hooks
·   All power tools should be 110V and provided with GFCI, Power source such as welding machine and portable generator shall be inspected by competent person, provided with grounding system and properly earthed.
·   Task supervisor to ensure all damage and uninspected power tools shall not be use and discard from working area
·   Prior to commence activity task supervisor should identify and determine and possible pinch point and corrective action shall be emphasized to all involve person.
·   Only competent fitters shall do the task and under the close supervision by their person in-charge such as supervisor or foreman.
·   Task person in-charge shall stop, correct unsafe condition and practice.
·   Prior to use of plant air secure valid approval from pre-com hydro test manager, only authorized person shall operate downstream valves after removing LOTO. Work area must be barricaded with signs.
·   Ensure all hoses are checked for correct pressure ratings, check for any signs of cracks and leaks, ensure whip lashed arrestor installed, Chicago clamp fittings installed with lock pins.
·   Use of jubilee clips are prohibited use only crimped type clamps to secure hose fittings
·   Proper work planning and proper coordination with other department working in same location.
·   Task supervisor to ensure adequate drinking water provided to workers and sufficient rest as per heat stress flagging system.

·   Noise monitoring shall be conducted and recorded as frequent as required by SSA HSE personnel.
·   Task supervisor to ensure all his work crew has readily available ear protection whenever working near high noise areas.
